The form prints out as 2 pages. We only need the second page when you come to play, so save a page of paper and print only pg. 2. (also see note below for another option)
Another way to get the form is to download either the Word document or PDF document that are posted below. Sharon Scott kindly did these for us on a single page. They seem to print out nicely in most cases even if your system indicates that the margins are too wide.
